In this course, you will learn practical ways to help patients find their best lifestyle fit treatment(s), assess them for clinical suitability, conduct home inspections, and empower your patients to take responsibility for as much of their own treatments as possible.

Objectives

  • Discuss the concept of “PD vs. HHD silos” and the impact of home therapies departments working across modalities to prioritize keeping home patients at home.
  • Explain how early education of PD patients about the potential temporary nature of PD and HHD as a next option may affect their Life Plans.
  • List three signs that a PD to HHD transition may be necessary in the near future.
  • Outline five steps to facilitate a change from PD to home HD.
  • Address needle fear as a barrier to a switch from PD to home HD.

Audience

Nurses and Dialysis Technicians. (This course can also serve as a home dialysis orientation for nephrologists and administrators.)
