In a culture of safety the focus is on effective systems and teamwork–including patients–to accomplish the mutual goal of safe, high-quality performance. When something goes wrong, the focus is on what, rather than who, is the problem. The intent is to bring process failures and system issues to light, and to solve them in a non-biased non-threatening way.
Objectives
After taking this course, participants will be able to:
- Name the key elements in facility safety
- List/Describe at least 3 system factors that impact patient care
- List at least 4 key members of the facility culture of safety team
- Describe the factors that hinder communication for a facility team
- Define “defect” and give an example in the dialysis facility
